Meet Our Staff

The Physicians, Hospice-trained Registered Nurses, Home Health Aides, Social Workers, Bereavement Coordinator, Volunteer Coordinator, Pastoral Care Team, Dietician, Therapists, Homemakers, and Volunteers of Hospice are all are trained to do Hospice work with the care and attention it so greatly deserves.

The staff members are patient and family advocates, assuring that care is based on the needs and desires of the patient and family.

Medical Director

The Medical Director is an Illinois licensed physician responsible for overseeing all the client’s Hospice care. The Director reviews each client’s Plan of Care to make sure all needs are being met in the best possible way.

Nursing Services

All Hospice nursing services are based on client need and performed under a “Plan of Care” for that client. All nurses are licensed professionals specifically trained to do Hospice nursing. In addition, each Home Health Aide is supervised by a Registered Nurse and is specifically trained to do Hospice Care.

Social Services

A Hospice Social Worker is assigned to each client and family for a psychological and social assessment. When indicated, a proper referral is made to the appropriate agency. The Social Worker documents the findings in the client chart and makes follow-up visits as indicated. When appropriate, client/family concerns are brought to the Interdisciplinary Team (see below).

Bereavement Coordinator

The Bereavement Coordinator facilitates all the Hospice bereavement services, including the monthly support group meetings for survivors and mailings to families and loved ones on a regular basis. The Coordinator works with client, families, and loved ones before and following the death.

Pastoral Director

Understanding that each individual has the right to his or her own spiritual beliefs, Hospice works with the client’s pastor or will attempt to find a pastor to meet the spiritual needs, if desired. The Hospice Pastoral Director and the client’s Pastor coordinate the care given to each family according to the wishes of each client and family.

Homemakers

Homemaker services are provided for light duty help within the home. These duties may include light laundry, dusting, mopping, meal preparation or vacuuming. Services are provided under the direction of the Nursing Supervisor.

Volunteers

Volunteers from all walks of life do many things for clients and their families. They listen, do light housework, cook, run errands, etc. Creative individuals utilize their talents in many ways including office work, work on special projects at fund-raisers, etc. Volunteers receive sixteen hours of instruction and frequent in-service seminars. Volunteers are an integral force in all we do.

Interdisciplinary Team

The Interdisciplinary Team consists of representatives from each discipline who meet bi-weekly to review the Plan of Care for each client. Each client is reviewed at this meeting, and changes are made in accordance with the needs and desires of the client, physician, and family. Recommended changes are proposed to the primary physician, client, and family.

Administrative Staff

The Administrative staff consists of the Executive Director, the Office Manager, the Accounting Manager, Administrative Assistant, Office Volunteers, and others. Each person is an important component of the Hospice team.
